Concern of the Patient

The patient came to us with a major dental problem that was not only affecting his confidence but was also disturbing his oral hygiene maintenance. This chief concern included crowding/misalignment of the teeth; he also had a crossbite teeth, which affected his bite and overall chewing efficiency. After a thorough clinical examination, we recorded a scan and developed a treatment plan using clear aligners.

Diagnosis

The patient was diagnosed with a Class II subdivision malocclusion associated with dental crowding and crossbite.

The primary treatment objectives were to:

  • Create adequate space for controlled alignment of the crowded teeth.
  • Correct the existing crossbite.
  • Establish improved dental alignment and interarch relationships.
  • Achieve a stable and functional occlusion through controlled tooth movement.

Customized Treatment Approach

The treatment involved carefully planned and sequential tooth movements using teeth aligners, supported by the following auxiliaries:

  • Interproximal Reduction (IPR): Selective reduction of enamel between specific teeth to create the space required for alignment while facilitating controlled tooth movement.
  • Optimized Attachments: Tooth-colored attachments were incorporated to provide additional grip and control, allowing the aligners to deliver more precise, predictable forces during treatment.
  • Temporary Bite Blocks: Bite-raising elements were incorporated into the aligners, which assisted in discluding the affected teeth and facilitating correction of the crossbite.
  • Minimal Expansion and Staging: Tooth movement was carefully sequenced, using minimal arch expansion, IPR, and staged individual movements to improve control throughout aligner therapy.

Treatment Progress

The treatment was completed in 12.5 months, with only three clinical visits. No refinement phase was required.
