Disclosures of services & treatment

The purpose of any orthodontic treatment is so that it can lead to a healthier and more beautiful smile. We, at toothsi, believe that for any orthodontic treatment to be successful: (1) t he treatment should be a partnership between toothsi and the desirous customer (yourself/ you/your) and (2) that informed and cooperative customer can achieve positive orthodontic results. And thus, before you begin any orthodontic treatment, you should also be aware that any such treatments may have some potential limitations and potential risks. Please read this information carefully. Ensure you know what is expected of you as the Customer (or as the parent/guardian of a young patient) during treatment.

About Aligners:

Toothsi aligners, comprise of a series of clear, removable aligners that gradually move the teeth to improve function along with the aesthetic appearance. Your customized toothsi Aligners are produced only after planning of your case using specialized computer technology by the planning team as per your requirement

Procedure:

Once you express an interest in availing the Aligners, you get a preliminary smile assessment done by our planning team basis your photos and information/data that you provide. A toothsi professional will take impressions/scans of your teeth and send them to the planning team or you can opt for your taking your own impressions under the guidance of a toothsi professional. On receipt of the above, our planning team will prepare a custom smile correction plan for you using computerized technology. This will be shared with you thus giving you an idea of the tooth movement that can be expected from the plan. Upon your approval of the plan prescribed, toothsi will produce and deliver a series of customized aligners to you, with in-clinic procedures scheduled as required. The total number of aligners will vary depending on the complexity of your case and the treatment plan. The aligners will be individually numbered and will be dispensed to you with specific instructions for use. Each customer is unique and treatment duration varies depending on the complexity of your case. Treatment duration may vary from the prescribed time in accordance with your response to the treatment. Some cases may require additional impressions/scans and/or refinement aligners after the initial series of aligners.

Expectations from You:

For the treatment to be successful and for the validation and guarantee of the “toothsi promise” to be applicable besides expected to adhere to some basic responsibilities and tasks including but not limited to flossing on a daily basis, being on time for scheduled clinic appointments, adhering to the food list prescribed, visiting your general dentist for regular six-month cleanings and check-up, you are also expected to use the clear aligners as we have instructed in the instruction manual shared with the toothsi kit and during the delivery visit, wear them for the full 22 hours a day, and for the duration recommended and follow up with us as instructed by sharing all records as well as the monitoring videos with the toothsi team and ensuring complete tracking of the approved plan.

As dental professionals we recommend that the customer take an OPG prior to the treatment to rule out any abnormalities. The customer can take the X-ray from any diagnostic center of your choice or any center prescribed by toothsi.

The aligners are clear, so people may not even notice you wearing them.
There are no cuts or abrasions from wires or brackets, so aligners are more comfortable than traditional braces.
Tooth movements can be viewed on your simulated treatment plan can be seen on the software
The aligners are removable, allowing you to eat, drink, and brush and ﬂoss with freedom.
Improved function of Chewing and biting.

Initial adjustment/acclimatization period

Dental pain or some discomfort may be experienced when moving to the next aligner in the series.
Gums, cheeks and lips may be scratched or irritated when removing the aligners. This is usually only experienced when learning to remove them in the first few days.
Like any dental treatment, teeth can still move or relapse after treatment. Regular wearing of retainers by the customer at the end of treatment should help prevent this.
The aligners may temporarily affect speech for the first few days/weeks. This is not permanent and you will adjust to wearing your aligners. It is common to experience a temporary increase in salivation or mouth dryness.
Your dentist may need to place attachments to one or more teeth during the course of treatment.
Teeth may require minor enamel reproximations or adjustments in order to create space to allow tooth movement to occur.

Potential Risks of Aligners

As with any other orthodontic treatments, the aligners may carry some of the potential risks described below:

Tooth decay, periodontal disease, inflammation of the gums or permanent markings (e.g. decalcification) may occur if patients consume foods or Beverages containing sugar whilst wearing the aligners.
General medical conditions and use of medications can affect orthodontic treatment.
Health of the bone and gums, which support the teeth, may be impaired or aggravated if regular professional care not taken.
Existing dental restorations (e.g. crowns) may become dislodged and require re-cementation, or in some instances, replacement as part of the treatment plan.
Excessively worn teeth can pose appliance retention issues and inhibit tooth movement.
The length of the roots of the teeth may be shortened during orthodontic treatment and may become a threat to the useful life of teeth.
Aligner breakage has a higher probability in cases with multiple missing teeth. Aligners or its parts thereof may be accidentally swallowed or aspirated.
Problems in the jaw joint, causing joint pain, headaches or ear problems can be experienced by patients undergoing aligner treatment.
Very rarely, allergic reactions may occur
Just like any medical /dental treatment- success of Smile correction with clear aligners depends on various factors such as compliance, customer cooperation, eating habits, biologic response of the tissues (bone, teeth, jaws) undergoing movement/change.
The treatment plan prescribed is prepared after evaluating specific requirements of the customer, corrections desired as communicated by customer, biological constraints of soft and hard tissues involved and orthodontic feasibility of aligners as an appliance to move teeth.
Complete /partial correct
ion with aligners may be possible depending on the case in question.
Failure to wear the aligners for the prescribed number of hours in the day and or not using the product as directed can lengthen the treatment time and affect the ability to achieve the desired results.

Toothsi Promise:

Toothsi is confident of both its products as well as their personnel, toothsi therefore assures its customers that if you do not have visibly straighter teeth despite following all instructions outlined in the instruction manual shared with the kit and during the delivery visit ,i.e wear them for the full 22 hours a day, for the duration recommended and follow up with us as instructed including sharing all records and updates as well as the monitoring videos with the toothsi team and ensuring complete tracking of the plan and adhering to all the expectations from you, you may be eligible to receive further aligners for refinement to achieve the optimum possible outcome based on your anatomy.

If you do not follow the instructions or do not wear the aligners as directed, the guarantee will also not apply. The guarantee refers to providing an individually achievable, straighter smile –as dictated by limits of biology and anatomy not “perfection”. It is clarified that after the smile correction plan has been approved by the customer, no refund of any monies paid for any product or service shall be made or rendered and nor shall the customer be eligible to claim the same.

Informed Consent & Agreement

I agree and confirm that I have read and understood all the content of the Disclosures describing the benefits, risks, alternatives and inconveniences associated with treatment as well as the option of no treatment of toothsi Aligners. All data and information I have required has been duly provided and The Informed Consent I render hereinbelow is made after full evaluation by me.

I have been duly intimated and understand that before initiation, that and in some cases prior to the treatment or during treatment the Customers may at toothsi’s discretion given the option to choose/opt for in-person consultation at their home or consultation via video and telephone.

I, hereby agree to provide the Toothsi representative with any required information, scans, study models, photographs or any other diagnostic aids he/she deems appropriate and requests to make a thorough diagnosis of my dental needs, treatment planning and progress tracking.

I have been given the opportunity to discuss and understand my smile plan. I have understood; and thereafter I consent, authorize and direct the planning team with associates or assistants of their choice to execute the proposed treatment plan that has been shared with me. I agree to follow the treatment exactly as planned, prescribed and provided for me.

I acknowledge that the plan provided to me enables me to understand the outcome I can expect with aligners. I understand that teeth movements beyond those provided in the plan will not be possible with the smile correction process undertaken by the assigned doctor and toothsi. Due to the fact that orthodontics is not an exact science, I acknowledge that my doctor nor my assigned dentist nor toothsi, its employees, representatives, successors, assigns, or agents, have, can or will make any promises or guarantees as to the success of my treatment or give any assurances of any kind concerning any particular result of my treatment.

I understand, acknowledge and agree that after the smile correction plan has been approved by me, no refund of any monies paid for any product or service shall be made or rendered and nor shall I be eligible to claim the same.

I recognize that any of the X-rays, impressions and photographs, reports, charts, medical history, photographs, findings, plaster models or impressions of teeth, prescriptions, diagnosis, medical testing, test results, billing, will be included in medical records, which records encompass “individually identifiable health information” as that term is defined and protected by the Data Protection laws. I further agree that toothsi may use such medical records/data for (i) disclosure to other licensed dentists or orthodontists and organizations employing licensed dentists and orthodontists and to toothsi, its representatives, employees, successors, assigns, and agents for the purposes of investigating and reviewing my medical history as it pertains to orthodontic treatment with the aligners (s) from toothsi and (ii) for educational and research purposes, but only to the extent that no individual identifiers, including but not limited to name or address, are thereby disclosed.

I hereby consent that any photographs taken of my face, jaws and teeth before, during and after treatment that may be used for dental records, dental education, demonstrations, and professional publications. They may also be used as marketing material for our website, patient education and printed materials (leaflets) but only to the extent that my name and other identifying information will be kept confidential unless I explicitly give my consent to the usage of the same without concealment. I hereby expressly duly consent to such uses and disclosure(s) as described herein.

I understand that toothsi is a platform that connects dental service providers and manufacturers of dental and Aligner products. Toothsi itself is also a manufacturer of dental and aligner products. Affiliated dental Service providers in charge of my treatment may or may not be wholly associated with Ampa orthodontics private limited. I understand toothsi is not a provider of dental, medical or health care services and cannot and does not practice dentistry, medicine or give medical advice. even though it may have dentists or orthodontists on payroll and/or under contract it does not render any warranty or guaranty in any manner.

I acknowledge I will not, nor shall anyone on my behalf, seek or obtain damages or remedies— legal, equitable, monetary or otherwise—arising from any of the disclosures that complies with the terms of this Informed Consent and Agreement.

I hereby expressly duly consent to all the above mentioned uses and disclosure(s) and accept the same as described herein.
